Field employees must be enrolled in the Hourbank plan for office staff to be eligible for this plan
Employees must work a minimum of 25 hours per week to participate
Employees must spend 40% or more of their work time in the office rather than in the field
Options
May choose 80% or 100% coverage for dental and prescriptions
May choose level of life insurance coverage from 1X, 2X or 3x annual income

Coverage for Office Employees
Life Insurance – 1X, 2X, or 3X annual earnings as selected by the employer to a maximum of $300,000, $10,000 for spouse and $5,000 for dependents
Accidental Death Insurance – same as life insurance coverage, payable on accidental death
Long Term Disability – 75%of monthly earnings if employer paid, 67% of the first $4000 of monthly income plus 50% of the balance up to a maximum benefit of $6000 per month if employee paid
Short Term Disability (OPTIONAL) – 75% of weekly income to $1100 if employer pays premiums, 67% if employee paid to a maximum of $1100
Prescription Drug Coverage – 80% or 100% coverage (employer elected) for most prescription drugs, no yearly limit. For those that choose the 80% option, this coverage increases to 90% once an individual has incurred prescription drug claims of $5,000 or more in a calendar year
Dental Care – 80% or 100% coverage (employer elected) of basic services, 50% coverage of major services, 50% coverage on orthodontics. Yearly limit of $2500 per person
Paramedical Practitioners – $500 maximum per year, per person, per practitioner such as naturopaths, chiropractors, physio therapists, massage therapists, speech therapists, etc.
Vision Care – $350 every 24 months for glasses or contact lens, $75 every 24 months for eye exams
Out-of-Country Travel – up to $2 million coverage for medical emergencies outside of Canada, limited to 60 days of travel per trip
Employee and Family Assistance Plan – toll free 24/7 access to counselling services
Best Doctors Coverage – access to world-renowned specialists’ second opinion for serious illness or injury diagnosis
